The undersigned applicant is hereby notified that Trio will obtain an investigative consumer report for application screening purposes. Such report may include information as to an applicant’s character, general reputation, personal characteristics, mode of living, history of criminal convictions, employment, credit, rental history and other such items. The undersigned applicant acknowledges their right to request a complete and accurate disclosure of the nature and scope of the investigation requested. Such a request will be mailed or otherwise delivered to applicant within a reasonable period of time from the date of the applicant's request for disclosure.
Pursuant to the United States and applicable state laws (collectively "federal and state law"), Trio certifies that the investigative consumer report requested to be supplied on the attached application is for screening purposes only; that Trio has complied with the disclosure requirements of federal and state law, as amended, by making clear and conspicuous disclosure in writing to the you that an investigative consumer report may be obtained for screening purposes; and that you are authorizing such procurement by your application submission. Pursuant of federal or state law, as amended, Trio nor its affiliate shall not use the investigative consumer report in violation of any other applicable federal or state fair housing, or equal opportunity law or regulation, and that Trio or its affiliates shall not take adverse action against you based in whole or in part of the investigative consumer report without providing the following contact information.

Each of the undersigned specifically represents to Trio and its affiliates, potential agents, brokers, processors, attorneys, insurers, servicers, successors and assigns and agrees and acknowledges that: (1) the information provided in this application is true and correct as of the date set forth opposite my signature and that any intentional or negligent misrepresentation of this information contained in this application may result in civil liability, including monetary damages, to any person who may suffer any loss due to reliance upon any misrepresentation that I have made on this application, and/or in criminal penalties including, but not limited to, fine or imprisonment or both under the provisions of Title 18, United States Code, Sec. 1001, et seq.; (2) the financing requested pursuant to this application will be or is already secured by a mortgage or deed of trust on the property described as an OwnOption Mortgage originated on my behalf; (3) the property will not be used for any illegal or prohibited purpose or use; (4) all statements made in this application are made for the purpose of obtaining a residential lease or rental agreement; (5) I intend to personally occupy the property and am not making application with the purposes of renting out the property; (6) Trio and its affiliates, servicers, successors or assigns may retain the original and/or an electronic record of this application, whether or not my financing is approved; (7) Trio and its agents, brokers, insurers, servicers, successors, and assigns may continuously rely on the information contained in the application, and I am obligated to amend and/or supplement the information provided in this application if any of the material facts that I have represented herein should change prior to my moving into the property; (8) in the event that my payments under my financing agreement become delinquent, Trio, its servicers, successors or assigns may, in addition to any other rights and remedies that it may have relating to such delinquency, report my name and account information to one or more consumer reporting agencies; and (9) my transmission of this application as an "electronic record" containing my " electronic signature," as those terms are defined in applicable federal and/or state laws (excluding audio and video recordings), or my facsimile transmission of this application containing a facsimile of my signature, shall be as effective, enforceable and valid as if a paper version of this application were delivered containing my original written signature. Each of the undersigned hereby acknowledges that Trio or its affiliates, its servicers, successors and assigns, may verify or reverify any information contained in this application or obtain any information or data relating to my application, for any legitimate business purpose through any source
